Once your application has been reviewed and approved you will receive a home study agreement form to sign and return along with your home study payment. You may then begin to download & complete the home study forms below.

RETURNING AAI FAMILIES: If you are requesting an update or amendment to your original home study or are returning for a subsequent home study, please download and the complete the home study update application. You are not required to submit an application fee. After your application has been reviewed and approved, and your home study agreement has been signed and returned, you may begin to download & complete forms 11-15.

16.	Child Protective Service Clearance* (PDF Only) - This form must be notarized and costs $5.00 per form. Note: They will NOT accept personal checks.
17.	Criminal Clearances* - Fingerprint cards will be provided by AAI. There is a $50 processing fee per card and must be paid to the Treasurer of Virginia in the form of a bank check or money order. They will NOT accept personal checks.
18.	Accuracy Statement/Permission to Finalize Report (Word document | PDF) Download and sign this form once all parties have reviewed the report and agree that it is ready to be printed and notarized.

* Note: Criminal Clearances & CPS Clearances are only required for new families and every three years for existing AAI families.
